Likely pathogenic for RYR1-related disorder — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000540.3(RYR1):c.15021G>C (p.Gln5007His), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces glutamine, which is neutral and polar, with histidine, which is basic and polar, at codon 5007 of the RYR1 protein (p.Gln5007His). This variant also falls at the last nucleotide of exon 105, which is part of the consensus splice site for this exon. This variant is present in population databases (rs376293495, gnomAD 0.003%). This variant has not been reported in the literature in individuals affected with RYR1-related conditions. ClinVar contains an entry for this variant (Variation ID: 1284912). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on RYR1 protein function. Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site.
