Uncertain significance for Dyskeratosis congenita, autosomal dominant 2; Idiopathic Pulmonary Fibrosis — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_198253.3(TERT):c.2315A>G (p.Tyr772Cys), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the TERT gene (transcript NM_198253.3) at coding-DNA position 2315, where A is replaced by G; at the protein level this means replaces tyrosine at residue 772 with cysteine — a missense variant. Submitter rationale: This sequence change replaces tyrosine, which is neutral and polar, with cysteine, which is neutral and slightly polar, at codon 772 of the TERT protein (p.Tyr772Cys). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of TERT-related conditions and/or idiopathic pulmonary fibrosis (PMID: 15814878; Invitae). ClinVar contains an entry for this variant (Variation ID: 12732). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt TERT protein function with a positive predictive value of 95%. Experimental studies have shown that this missense change affects TERT function (PMID: 15814878, 23901009). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
