Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_004958.4(MTOR):c.1816T>C (p.Cys606Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the MTOR gene (transcript NM_004958.4) at coding-DNA position 1816, where T is replaced by C; at the protein level this means replaces cysteine at residue 606 with arginine — a missense variant. Submitter rationale: This sequence change replaces cysteine, which is neutral and slightly polar, with arginine, which is basic and polar, at codon 606 of the MTOR protein (p.Cys606Arg). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of Smith-Kingsmore syndrome (PMID: 39030910). In at least one individual the variant was observed to be de novo. ClinVar contains an entry for this variant (Variation ID: 1213694). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt MTOR protein function with a negative predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
