This variant lies in the TP53 gene (transcript NM_000546.6) at the canonical splice donor site of the intron immediately after coding-DNA position 672, where T is replaced by C; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.672+2T>C intronic variant results from a T to C substitution two nucleotides after coding exon 5 in the TP53 gene. This variant was reported in individual(s) with features consistent with Li-Fraumeni syndrome (Gonzalez KD et al. J Med Genet, 2009 Oct;46:689-93). This variant was detected in at least one individual at an allele fraction that is suggestive of clonal hematopoiesis, a predictor of TP53 pathogenicity (Ambry internal data; Fortuno C et al. Genet Med. 2022 03;24:673-680). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site and may result in the creation or strengthening of a novel splice donor site. Alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. As such, this alteration is classified as likely pathogenic.
