This variant lies in the ALMS1 gene (transcript NM_001378454.1) at coding-DNA position 1966, where G is replaced by T; at the protein level this means converts the codon for glutamic acid at residue 656 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: Variant summary: ALMS1 c.1963G>T (p.Glu655X) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, which are commonly known mechanisms for disease. Truncations downstream of this position have been classified as pathogenic by our laboratory. The variant was absent in 248596 control chromosomes. To our knowledge, no occurrence of c.1963G>T in individuals affected with Alstrom Syndrome With Dilated Cardiomyopathy and no experimental evidence demonstrating its impact on protein function have been reported. However, a strong clustering of other truncating variants in exon 8 of the ALMS1 gene has been reported in patients with Alstrom syndrome (Marshall_2015 and the HGMD database). No clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014.
