Pathogenic — the classification assigned by Illumina Laboratory Services, Illumina to GRCh37/hg19 1q42.2-43(chr1:231407943-237289859)x1, citing ICSL CNVClassificationCriteria Jul2020Prior. This is a single-copy loss (one copy instead of two) of the chr1:231407943-237289859 region (~5.88 Mb) on cytogenetic band 1q42.2-43. Submitter rationale: This CNV is a 5.8 Mb deletion of 1q42.2-1q43 on chromosome 1, (seq[GRCh37]del(1)(q42.2q43); chr1:g.231407943_237289859del) which has been found in a de novo state. The CNV constitutes a loss encompassing 55 genes and partially overlaps deletions in 1q4 that have been described in the literature (De Vries et al. 2001; Kanemoto et al. 2006; CÃ³rdova-Fletes et al. 2008; Radha Rama Devi et al. 2019). In addition, a deletion fully encompassed within the CNV observed in the proband has been described in DECIPHER (Firth et al. 2009). The 1q4 deletions, which typically occur de novo, are diverse in terms of size and breakpoints and are associated with phenotypic variability. The primary features include developmental delay, intellectual disability, seizures, microcephaly, and midline defects such as cleft palate and corpus callosum abnormalities. Dysmorphic facial features most commonly include deep-set eyes, epicanthal folds, round face with prominent forehead, flat nasal bridge, and short nose with long philtrum. Additional features seen in some affected individuals include heart anomalies, urogenital anomalies, skeletal defects, and strabismus. While interstitial 1q41q42 deletions and 1q43q44 terminal deletions have been more commonly reported to date, non-terminal 1q4 deletions that spare q41 have also been reported (Kanemoto et al. 2006; CÃ³rdova-Fletes et al. 2008; Firth et al. 2009; Radha Rama Devi et al. 2019). Deletions of a similar size have not been reported in controls (Cooper et al. 2011; MacDonald et al. 2014). Based on the collective evidence, this CNV is classified as pathogenic.
