Uncertain significance for Mendelian susceptibility to mycobacterial diseases due to complete ISG15 deficiency — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_005101.4(ISG15):c.95T>A (p.Ile32Asn), citing Invitae Variant Classification Sherloc (09022015): This variant has not been reported in the literature in individuals with ISG15-related conditions. This sequence change replaces isoleucine with asparagine at codon 32 of the ISG15 protein (p.Ile32Asn). The isoleucine residue is highly conserved and there is a large physicochemical difference between isoleucine and asparagine. This variant is not present in population databases (ExAC no frequency). Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
