Pathogenic for Adrenoleukodystrophy — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000033.4(ABCD1):c.1165C>G (p.Arg389Gly), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the ABCD1 gene (transcript NM_000033.4) at coding-DNA position 1165, where C is replaced by G; at the protein level this means replaces arginine at residue 389 with glycine — a missense variant. Submitter rationale: For these reasons, this variant has been classified as Pathogenic. This variant disrupts the p.Arg389 amino acid residue in ABCD1. Other variant(s) that disrupt this residue have been observed in individuals with ABCD1-related conditions (PMID: 7825602, 15811009), which suggests that this may be a clinically significant amino acid residue. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C45"). ClinVar contains an entry for this variant (Variation ID: 11296). This missense change has been observed in individual(s) with adrenomyeloneuropathy (PMID: 8566952, 15811009, 22479560; Invitae). In at least one individual the variant was observed to be de novo. This variant is not present in population databases (gnomAD no frequency). This sequence change replaces arginine, which is basic and polar, with glycine, which is neutral and non-polar, at codon 389 of the ABCD1 protein (p.Arg389Gly).
