Likely pathogenic for Cardiovascular phenotype; Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_001042492.3(NF1):c.2409+2T>A, citing Ambry Variant Classification Scheme 2023. This variant lies in the NF1 gene (transcript NM_001042492.3) at the canonical splice donor site of the intron immediately after coding-DNA position 2409, where T is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.2409+2T>A intronic variant results from a T to A substitution two nucleotides after coding exon 20 in the NF1 gene. In silico splice site analysis predicts that this alteration will weaken the native splice donor site. A resulting transcript is predicted to be in-frame and is not expected to trigger nonsense-mediated mRNA decay, the region predicted to be impacted is critical for protein function (Ambry internal data). RNA studies have demonstrated that this alteration results in abnormal splicing in the set of samples tested (Ambry internal data). This variant was reported in individual(s) with features consistent with neurofibromatosis type 1 (NF1) (Ambry internal data). Other alterations impacting the same donor site (c.2409+1G>C) have also been identified in individuals with a clinical diagnosis or suspicion of NF1 (Bausch B, J. Clin. Endocrinol. Metab. 2007 Jul; 92(7):2784-92). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). Based on the majority of available evidence to date, this variant is likely to be pathogenic.
