This variant lies in the FH gene (transcript NM_000143.4) at coding-DNA position 1499, where G is replaced by A; at the protein level this means converts the codon for tryptophan at residue 500 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: This sequence change creates a premature translational stop signal (p.Trp500*) in the FH gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 11 amino acid(s) of the FH protein. This variant is not present in population databases (gnomAD no frequency). This premature translational stop signal has been observed in individual(s) with fumarate hydratase deficiency and/or hereditary leiomyomatosis and renal cell cancer (PMID: 9635293, 21398687). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. This variant is also known as G1373A (W458X). ClinVar contains an entry for this variant (Variation ID: 1076641). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. For these reasons, this variant has been classified as Pathogenic.
