This variant lies in the ENG gene (transcript NM_001114753.3) at the canonical splice donor site of the intron immediately after coding-DNA position 523, where G is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.523+1G>A intronic variant results from a G to A substitution one nucleotide after coding exon 4 of the ENG gene. In our internal cohort, this alteration was identified in an individual with a clinical diagnosis of hereditary hemorrhagic telangiectasia. This nucleotide position is highly conserved in available vertebrate species. Using the BDGP and ESEfinder splice site prediction tools, this alteration is predicted to abolish the native splice donor site; however, direct evidence is unavailable. Alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. As such, this alteration is classified as likely pathogenic.
