Pathogenic for Hyperkalemic periodic paralysis — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000334.4(SCN4A):c.608T>A (p.Met203Lys), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces methionine, which is neutral and non-polar, with lysine, which is basic and polar, at codon 203 of the SCN4A protein (p.Met203Lys). This variant is present in population databases (no rsID available, gnomAD 0.0009%). This missense change has been observed in individual(s) with autosomal recessive SCN4A-related conditions (PMID: 26700687). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 1074599). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on SCN4A protein function. Experimental studies have shown that this missense change affects SCN4A function (PMID: 26700687). For these reasons, this variant has been classified as Pathogenic.
