Pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NC_000023.10:g.(?_107913448)_(107939618_?)del, citing Invitae Variant Classification Sherloc (09022015): Glycine residues within the Gly-Xaa-Yaa repeats of the triple helix domain are required for the structure and stability of fibrillar collagens (PMID: 7695699, 8218237, 19344236). In COL4A5, missense variants at these glycine residues are significantly enriched in individuals with disease (PMID: 23720012, 27627812) compared to the general population (ExAC). For these reasons, this variant has been classified as Pathogenic. This variant has not been reported in the literature in individuals with COL4A5-related conditions. This variant is a gross deletion of the genomic region encompassing exons 42-51 of the COL4A5 gene. The 5' boundary is likely confined to intron 41. The 3' end of this event is unknown as it extends through the termination codon beyond the assayed region for this gene and may encompass additional genes. While this deletion is not anticipated to result in nonsense mediated decay, it is expected to create a truncated protein product or disrupt mRNA translation.
