This deletion is predicted to remove amino acids 381-552 of the SMAD4 protein, constituting approximately 33% of the protein. This will result in the loss of most of the mad homology 2 region (MH2, residues 323-552) of the SMAD4 protein, which is expected to interfere with the homo-oligomer formation of SMAD4 and the hetero-oligomer formation between SMAD4 and SMAD2, resulting in disruption of TGFŒ≤ signaling (PMID: 9389648, 12821112), While this particular deletion has not been reported in the literature, loss-of-function variants in SMAD4 are known to be pathogenic (PMID: 22810475, 16152648). This variant is a gross deletion of the genomic region encompassing exons 10-12 of the SMAD4 gene. The 5' boundary is likely confined to intron 9. The 3' end of this event is unknown as it extends through the termination codon beyond the assayed region for this gene and may encompass additional genes. While this deletion is not anticipated to result in nonsense mediated decay, it is expected to create a truncated SMAD4 protein.
