Likely pathogenic for 6-Pyruvoyl-tetrahydrobiopterin synthase deficiency — the classification assigned by Women's Health and Genetics/Laboratory Corporation of America, LabCorp to NM_000317.3(PTS):c.104A>G (p.Glu35Gly), citing LabCorp Variant Classification Summary - May 2015. This variant lies in the PTS gene (transcript NM_000317.3) at coding-DNA position 104, where A is replaced by G; at the protein level this means replaces glutamic acid at residue 35 with glycine — a missense variant. Submitter rationale: Variant summary: PTS c.104A>G (p.Glu35Gly) results in a non-conservative amino acid change in the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 245486 control chromosomes (gnomAD v2.1). c.104A>G has been reported in the literature in compound heterozygous state in individuals affected with 6-Pyruvoyl-Tetrahydropterin Synthase Deficiency (Zekanowski_1998, Jaggi_2008), and in one of these cases it was observed in trans with a pathogenic variant (Jaggi_2008). These data indicate that the variant is likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. One clinical diagnostic laboratory has submitted clinical-significance assessments for this variant to ClinVar after 2014, and classified the variant as likely pathogenic. Based on the evidence outlined above, the variant was classified as likely pathogenic.
