This variant lies in the GBE1 gene (transcript NM_000158.4) at the canonical splice donor site of the intron immediately after coding-DNA position 992, where T is replaced by G; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.992+2T>G variant in GBE1 has not been previously reported in the literature in individuals with GBE1-related disorders, but has been identified in 0.002% (1/59912) of Admixed American chromosomes by the Genome Aggregation Database (gnomAD, http://gnomad.broadinstitute.org; dbSNP ID: rs772349876). Although this variant has been seen in the general population in a heterozygous state, its frequency is low enough to be consistent with a recessive carrier frequency. This variant has also been reported in ClinVar (Variation ID#: 1066789) and has been interpreted as likely pathogenic by Baylor Genetics and Labcorp Genetics. This variant is located in the 3' splice region. SpliceAI predictions indicate use of an out-of-frame cryptic splice site 5 bases from the intron-exon boundary, providing evidence that this variant may cause a frameshift and lead to a premature termination codon downstream. This alteration is then predicted to lead to a truncated or absent protein. However, this information is not predictive enough to determine pathogenicity.
