Uncertain significance for 3-Methylglutaconic aciduria type 3; Optic atrophy 3 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_025136.4(OPA3):c.206_258del (p.Lys69fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the OPA3 gene (transcript NM_025136.4) at coding-DNA position 206 through coding-DNA position 258, deleting 53 bases; at the protein level this means shifts the reading frame starting at lysine residue 69, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This sequence change creates a premature translational stop signal (p.Lys69Serfs*110) in the OPA3 gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 111 amino acid(s) of the OPA3 protein. This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with OPA3-related conditions. ClinVar contains an entry for this variant (Variation ID: 1066277). This variant disrupts the C-terminus of the OPA3 protein. Other variant(s) that disrupt this region (p.Q139*) have been observed in individuals with OPA3-related conditions (PMID: 18985435). This suggests that this may be a clinically significant region of the protein. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
