NM_001330078.2(NRXN1):c.1750G>A (p.Gly584Arg) was classified as Uncertain significance for Pitt-Hopkins-like syndrome 2 by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces glycine with arginine at codon 624 of the NRXN1 protein (p.Gly624Arg). The glycine residue is highly conserved and there is a moderate physicochemical difference between glycine and arginine. This variant is present in population databases (rs749913654, ExAC 0.01%). This variant has not been reported in the literature in individuals with NRXN1-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
