Uncertain significance for Pulmonary hypertension, primary, 2 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001127217.3(SMAD9):c.449A>G (p.Tyr150Cys), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces tyrosine with cysteine at codon 150 of the SMAD9 protein (p.Tyr150Cys). The tyrosine residue is weakly conserved and there is a large physicochemical difference between tyrosine and cysteine. This variant is present in population databases (rs138822893, ExAC 0.02%), and has an allele count higher than expected for a pathogenic variant (PMID: 28166811). This variant has not been reported in the literature in individuals affected with SMAD9-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Benign"; Align-GVGD: "Class C25"). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
