Uncertain significance for Aortic aneurysm, familial thoracic 4 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_002474.3(MYH11):c.3249G>A (p.Met1083Ile), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the MYH11 gene (transcript NM_002474.3) at coding-DNA position 3249, where G is replaced by A; at the protein level this means replaces methionine at residue 1083 with isoleucine — a missense variant. Submitter rationale: This sequence change replaces methionine with isoleucine at codon 1090 of the MYH11 protein (p.Met1090Ile). The methionine residue is highly conserved and there is a small physicochemical difference between methionine and isoleucine. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has not been reported in the literature in individuals with MYH11-related conditions. This variant is not present in population databases (ExAC no frequency). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
