Uncertain significance for Catecholaminergic polymorphic ventricular tachycardia 1 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001035.3(RYR2):c.12620C>T (p.Ser4207Leu), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces serine with leucine at codon 4207 of the RYR2 protein (p.Ser4207Leu). The serine residue is highly conserved and there is a large physicochemical difference between serine and leucine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with RYR2-related conditions. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt RYR2 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
