Uncertain significance for Gorlin syndrome; Medulloblastoma — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_016169.4(SUFU):c.815A>G (p.Lys272Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the SUFU gene (transcript NM_016169.4) at coding-DNA position 815, where A is replaced by G; at the protein level this means replaces lysine at residue 272 with arginine — a missense variant. Submitter rationale: This sequence change replaces lysine with arginine at codon 272 of the SUFU protein (p.Lys272Arg). The lysine residue is highly conserved and there is a small physicochemical difference between lysine and arginine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with SUFU-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site, but this prediction has not been confirmed by published transcriptional studies. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
