Uncertain significance for Microcephaly-intellectual disability-sensorineural hearing loss-epilepsy-abnormal muscle tone syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_145207.3(AFG2A):c.1010T>C (p.Ile337Thr), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the AFG2A gene (transcript NM_145207.3) at coding-DNA position 1010, where T is replaced by C; at the protein level this means replaces isoleucine at residue 337 with threonine — a missense variant. Submitter rationale: This sequence change replaces isoleucine with threonine at codon 337 of the SPATA5 protein (p.Ile337Thr). The isoleucine residue is moderately conserved and there is a moderate physicochemical difference between isoleucine and threonine. This variant is present in population databases (rs775914629, ExAC 0.01%). This variant has not been reported in the literature in individuals affected with SPATA5-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
