Uncertain significance for Sterile multifocal osteomyelitis with periostitis and pustulosis — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_173842.3(IL1RN):c.500T>A (p.Met167Lys), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the IL1RN gene (transcript NM_173842.3) at coding-DNA position 500, where T is replaced by A; at the protein level this means replaces methionine at residue 167 with lysine — a missense variant. Submitter rationale: This variant is present in population databases (rs765957403, ExAC 0.002%). This sequence change replaces methionine with lysine at codon 170 of the IL1RN protein (p.Met170Lys). The methionine residue is weakly conserved and there is a moderate physicochemical difference between methionine and lysine. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function output the following: SIFT: "Tolerated"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0". The lysine amino acid residue is found in multiple mammalian species, suggesting that this missense change does not adversely affect protein function. These predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has not been reported in the literature in individuals with IL1RN-related conditions.
