Uncertain significance for Idiopathic generalized epilepsy; Hyperaldosteronism, familial, type IV — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_021098.3(CACNA1H):c.412G>A (p.Ala138Thr), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the CACNA1H gene (transcript NM_021098.3) at coding-DNA position 412, where G is replaced by A; at the protein level this means replaces alanine at residue 138 with threonine — a missense variant. Submitter rationale: This sequence change replaces alanine, which is neutral and non-polar, with threonine, which is neutral and polar, at codon 138 of the CACNA1H protein (p.Ala138Thr). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with CACNA1H-related conditions. ClinVar contains an entry for this variant (Variation ID: 1050998). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
