Uncertain significance — the classification assigned by Department of Pathology and Laboratory Medicine, Sinai Health System to NM_005235.3(ERBB4):c.421+5G>A. This variant lies in the ERBB4 gene (transcript NM_005235.3) at 5 bases into the intron immediately after coding-DNA position 421, where G is replaced by A. Submitter rationale: The ERBB4 c.421+5G>A variant was not identified in the literature nor was it identified in ClinVar or LOVD 3.0. The variant was identified in dbSNP (ID: rs778195807) and in control databases in 8 of 250440 chromosomes at a frequency of 0.000032 (Genome Aggregation Database March 6, 2019, v2.1.1). The variant was observed in the European (non-Finnish) population in 8 of 113080 chromosomes (freq: 0.000071), but was not observed in the African, Latino, Ashkenazi Jewish, East Asian, European (Finnish), Other, or South Asian populations. The c.421+5G>A variant is located in the 5' splice region but does not affect the invariant +1 and +2 positions. However, positions +3 to +6 are part of the splicing consensus sequence and variants involving these positions sometimes affect splicing. Three of four in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer) predict a greater than 10% difference in splicing; however RNA analysis has not been performed therefore this is not very predictive of pathogenicity. In summary, based on the above information the clinical significance of this variant cannot be determined with certainty at this time. This variant is classified as a variant of uncertain significance.
