Uncertain significance — the classification assigned by Department of Pathology and Laboratory Medicine, Sinai Health System to NM_006922.4(SCN3A):c.774G>C (p.Leu258=): The SCN3A p.Leu258Leu variant was not identified in the literature nor was it identified in dbSNP, ClinVar or LOVD 3.0. The variant was identified in Cosmic (FATHMM predicted pathogenic; score=0.80) but was not identified in the following control databases: the 1000 Genomes Project, the NHLBI GO Exome Sequencing Project, the Exome Aggregation Consortium (August 8th 2016), or the Genome Aggregation Database (Feb 27, 2017). The p.Leu258Leu variant is not expected to have clinical significance because it does not result in a change of amino acid and is not located in a known consensus splice site. The variant occurs outside of the splicing consensus sequence however 4 of 4 in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer) predict a greater than 10% difference in splicing and the creation of a new 3' splice site. However, this information is not predictive enough to assume pathogenicity. In summary, based on the above information the clinical significance of this variant cannot be determined with certainty at this time. This variant is classified as a variant of uncertain significance.
