Uncertain significance for Holoprosencephaly 5 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_007129.5(ZIC2):c.450C>G (p.His150Gln), citing Invitae Variant Classification Sherloc (09022015): This variant has not been reported in the literature in individuals with ZIC2-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0"). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site, but this prediction has not been confirmed by published transcriptional studies. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. This sequence change replaces histidine with glutamine at codon 150 of the ZIC2 protein (p.His150Gln). The histidine residue is highly conserved and there is a small physicochemical difference between histidine and glutamine. The frequency data for this variant in the population databases is considered unreliable, as metrics indicate insufficient coverage at this position in the ExAC database.
