Uncertain significance for Symmetrical dyschromatosis of extremities; Aicardi-Goutieres syndrome 6 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001111.5(ADAR):c.3464G>A (p.Arg1155Gln), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the ADAR gene (transcript NM_001111.5) at coding-DNA position 3464, where G is replaced by A; at the protein level this means replaces arginine at residue 1155 with glutamine — a missense variant. Submitter rationale: This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 1155 of the ADAR protein (p.Arg1155Gln). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with ADAR-related conditions. ClinVar contains an entry for this variant (Variation ID: 1041359). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on ADAR protein function. This variant disrupts the p.Arg1155 amino acid residue in ADAR. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 15659327, 22336994). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
