NM_017636.4(TRPM4):c.1263+1G>A was classified as Uncertain significance for Cardiovascular phenotype by Ambry Genetics, citing Ambry Variant Classification Scheme 2023. This variant lies in the TRPM4 gene (transcript NM_017636.4) at the canonical splice donor site of the intron immediately after coding-DNA position 1263, where G is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.1263+1G>A intronic variant results from a G to A substitution one nucleotide after coding exon 10 of the TRPM4 gene. This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site and will result in the creation or strengthening of a novel splice donor site. Alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. However, loss of function of TRPM4 has not been clearly established as a mechanism of disease. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
