Pathogenic for Leber congenital amaurosis 8; Retinitis pigmentosa 12 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_201253.3(CRB1):c.1522T>C (p.Cys508Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the CRB1 gene (transcript NM_201253.3) at coding-DNA position 1522, where T is replaced by C; at the protein level this means replaces cysteine at residue 508 with arginine — a missense variant. Submitter rationale: This variant is not present in population databases (gnomAD no frequency). This sequence change replaces cysteine, which is neutral and slightly polar, with arginine, which is basic and polar, at codon 508 of the CRB1 protein (p.Cys508Arg). This missense change has been observed in individual(s) with retinal dystrophy (PMID: 31630094; Invitae). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. For these reasons, this variant has been classified as Pathogenic. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt CRB1 protein function. ClinVar contains an entry for this variant (Variation ID: 1039551).
