Uncertain significance for Cryptosporidiosis-chronic cholangitis-liver disease syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_181078.3(IL21R):c.1364A>T (p.Asp455Val), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces aspartic acid, which is acidic and polar, with valine, which is neutral and non-polar, at codon 455 of the IL21R protein (p.Asp455Val). This variant is present in population databases (rs370088512, gnomAD 0.005%). This variant has not been reported in the literature in individuals affected with IL21R-related conditions. ClinVar contains an entry for this variant (Variation ID: 1038095). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt IL21R protein function with a negative predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
