Uncertain significance for Neuronopathy, distal hereditary motor, type 7A; Congenital myasthenic syndrome 20 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_021815.5(SLC5A7):c.949G>T (p.Asp317Tyr), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces aspartic acid with tyrosine at codon 317 of the SLC5A7 protein (p.Asp317Tyr). The aspartic acid residue is highly conserved and there is a large physicochemical difference between aspartic acid and tyrosine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with SLC5A7-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
