Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_021102.4(SPINT2):c.553C>T (p.Arg185Cys), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces arginine with cysteine at codon 185 of the SPINT2 protein (p.Arg185Cys). The arginine residue is weakly conserved and there is a large physicochemical difference between arginine and cysteine. This variant is present in population databases (rs781030322, ExAC 0.01%). This variant has not been reported in the literature in individuals affected with SPINT2-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C15"). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
