Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000264.5(PTCH1):c.3911G>A (p.Arg1304Lys), citing Ambry Variant Classification Scheme 2023. This variant lies in the PTCH1 gene (transcript NM_000264.5) at coding-DNA position 3911, where G is replaced by A; at the protein level this means replaces arginine at residue 1304 with lysine — a missense variant. Submitter rationale: The p.R1304K variant (also known as c.3911G>A), located in coding exon 23 of the PTCH1 gene, results from a G to A substitution at nucleotide position 3911. This alteration was identified in 1/1358 non-cancer control individuals and in 0/57 cases, in a study looking at cancer predisposition mutations in patients with cutaneous melanoma and a history of at least two additional non-cutaneous melanoma primary cancers (Pritchard AL et al. PLoS One, 2018 Apr;13:e0194098). The arginine at codon 1304 is replaced by lysine, an amino acid with highly similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear.
