Uncertain significance for Rolandic epilepsy, intellectual disability, and speech dyspraxia, X-linked — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_014467.3(SRPX2):c.553C>T (p.Arg185Cys), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces arginine with cysteine at codon 185 of the SRPX2 protein (p.Arg185Cys). The arginine residue is highly conserved and there is a large physicochemical difference between arginine and cysteine. This variant is present in population databases (rs370080258, ExAC 0.01%). This variant has not been reported in the literature in individuals affected with SRPX2-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
