NM_000270.4(PNP):c.337G>A (p.Val113Ile) was classified as Uncertain significance for Purine-nucleoside phosphorylase deficiency by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces valine with isoleucine at codon 113 of the PNP protein (p.Val113Ile). The valine residue is highly conserved and there is a small physicochemical difference between valine and isoleucine. This variant is present in population databases (rs745437851, ExAC 0.07%). This variant has not been reported in the literature in individuals affected with PNP-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
