Uncertain significance for Periodic fever-infantile enterocolitis-autoinflammatory syndrome; Familial cold autoinflammatory syndrome 4 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001199138.2(NLRC4):c.1754A>C (p.Tyr585Ser), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the NLRC4 gene (transcript NM_001199138.2) at coding-DNA position 1754, where A is replaced by C; at the protein level this means replaces tyrosine at residue 585 with serine — a missense variant. Submitter rationale: This sequence change replaces tyrosine with serine at codon 585 of the NLRC4 protein (p.Tyr585Ser). The tyrosine residue is highly conserved and there is a large physicochemical difference between tyrosine and serine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with NLRC4-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C15"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
