Uncertain significance for Autosomal recessive distal spinal muscular atrophy 1; Charcot-Marie-Tooth disease axonal type 2S — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_002180.3(IGHMBP2):c.2611G>A (p.Gly871Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the IGHMBP2 gene (transcript NM_002180.3) at coding-DNA position 2611, where G is replaced by A; at the protein level this means replaces glycine at residue 871 with arginine — a missense variant. Submitter rationale: This sequence change replaces glycine with arginine at codon 871 of the IGHMBP2 protein (p.Gly871Arg). The glycine residue is moderately conserved and there is a moderate physicochemical difference between glycine and arginine. This variant also falls at the last nucleotide of exon 13, which is part of the consensus splice site for this exon. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals affected with IGHMBP2-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
