Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_006214.4(PHYH):c.658C>T (p.His220Tyr), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces histidine with tyrosine at codon 220 of the PHYH protein (p.His220Tyr). The histidine residue is highly conserved and there is a moderate physicochemical difference between histidine and tyrosine. This variant is present in population databases (rs767216891, ExAC 0.006%). This missense change has been observed in individual(s) with clinical features of Refsum disease (PMID: 14974078). In at least one individual the data is consistent with the variant being in trans (on the opposite chromosome) from a pathogenic variant. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
