Uncertain significance for Jeune thoracic dystrophy; Nephronophthisis — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_024753.5(TTC21B):c.1768A>G (p.Met590Val), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces methionine with valine at codon 590 of the TTC21B protein (p.Met590Val). The methionine residue is moderately conserved and there is a small physicochemical difference between methionine and valine. This variant is present in population databases (rs777733112, ExAC 0.01%). This variant has not been reported in the literature in individuals affected with TTC21B-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
