Uncertain significance for Tall stature-scoliosis-macrodactyly of the great toes syndrome; Acromesomelic dysplasia 1, Maroteaux type — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_003995.4(NPR2):c.1844T>G (p.Leu615Trp), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the NPR2 gene (transcript NM_003995.4) at coding-DNA position 1844, where T is replaced by G; at the protein level this means replaces leucine at residue 615 with tryptophan — a missense variant. Submitter rationale: This sequence change replaces leucine, which is neutral and non-polar, with tryptophan, which is neutral and slightly polar, at codon 615 of the NPR2 protein (p.Leu615Trp). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with NPR2-related conditions. ClinVar contains an entry for this variant (Variation ID: 1019606). Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
