Uncertain significance for Idiopathic generalized epilepsy; Hyperaldosteronism, familial, type IV — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_021098.3(CACNA1H):c.2478G>C (p.Glu826Asp), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the CACNA1H gene (transcript NM_021098.3) at coding-DNA position 2478, where G is replaced by C; at the protein level this means replaces glutamic acid at residue 826 with aspartic acid — a missense variant. Submitter rationale: This sequence change replaces glutamic acid with aspartic acid at codon 826 of the CACNA1H protein (p.Glu826Asp). The glutamic acid residue is highly conserved and there is a small physicochemical difference between glutamic acid and aspartic acid. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with CACNA1H-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
