Uncertain Significance for Malignant hyperthermia, susceptibility to, 1 — the classification assigned by All of Us Research Program, National Institutes of Health to NM_000540.3(RYR1):c.1411C>T (p.Arg471Cys), citing ACMG Guidelines, 2015. This variant lies in the RYR1 gene (transcript NM_000540.3) at coding-DNA position 1411, where C is replaced by T; at the protein level this means replaces arginine at residue 471 with cysteine — a missense variant. Submitter rationale: This missense variant replaces arginine with cysteine at codon 471 of the RYR1 protein, in a region considered to be a hotspot for pathogenic variants that contribute to malignant hyperthermia susceptibility (PMID: 21118704). Computational prediction is inconclusive regarding the impact of this variant on protein structure and function. To our knowledge, functional studies have not been reported for this variant. This variant has been reported in two families affected with malignant hyperthermia susceptibility; however, these families each carried a known pathogenic variant, p.Gly248Arg and p.Arg552Trp, that segregated with disease (PMID: 9138151, 1354642). This variant has been identified in 1/248190 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
