Uncertain significance for Marfan syndrome; Familial thoracic aortic aneurysm and aortic dissection — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000138.5(FBN1):c.4193A>G (p.Asp1398Gly), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the FBN1 gene (transcript NM_000138.5) at coding-DNA position 4193, where A is replaced by G; at the protein level this means replaces aspartic acid at residue 1398 with glycine — a missense variant. Submitter rationale: This sequence change replaces aspartic acid with glycine at codon 1398 of the FBN1 protein (p.Asp1398Gly). The aspartic acid residue is highly conserved and there is a moderate physicochemical difference between aspartic acid and glycine. This variant is present in population databases (rs781045902, ExAC 0.002%). This variant has not been reported in the literature in individuals affected with FBN1-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C65"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
