Likely pathogenic for Seizures, benign familial infantile, 3; Developmental and epileptic encephalopathy, 11 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001040142.2(SCN2A):c.2305A>G (p.Ile769Val), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the SCN2A gene (transcript NM_001040142.2) at coding-DNA position 2305, where A is replaced by G; at the protein level this means replaces isoleucine at residue 769 with valine — a missense variant. Submitter rationale: This sequence change replaces isoleucine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 769 of the SCN2A protein (p.Ile769Val). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. This variant disrupts the p.Ile769 amino acid residue in SCN2A. Other variant(s) that disrupt this residue have been observed in individuals with SCN2A-related conditions (PMID: 29314583), which suggests that this may be a clinically significant amino acid residue. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). ClinVar contains an entry for this variant (Variation ID: 1014704). This missense change has been observed in individual(s) with clinical features of SCN2A-related conditions (PMID: 33240318; Invitae). In at least one individual the variant was observed to be de novo. This variant is not present in population databases (gnomAD no frequency).
