Uncertain significance for Pulmonary fibrosis and/or bone marrow failure, Telomere-related, 3; Dyskeratosis congenita, autosomal recessive 5 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001283009.2(RTEL1):c.2692C>T (p.Leu898Phe), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the RTEL1 gene (transcript NM_001283009.2) at coding-DNA position 2692, where C is replaced by T; at the protein level this means replaces leucine at residue 898 with phenylalanine — a missense variant. Submitter rationale: This sequence change replaces leucine, which is neutral and non-polar, with phenylalanine, which is neutral and non-polar, at codon 898 of the RTEL1 protein (p.Leu898Phe). This variant is present in population databases (rs555482478, gnomAD 0.006%). This variant has not been reported in the literature in individuals affected with RTEL1-related conditions. ClinVar contains an entry for this variant (Variation ID: 1008329). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
