Uncertain significance for Hyperaldosteronism, familial, type IV; Idiopathic generalized epilepsy — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_021098.3(CACNA1H):c.1364G>A (p.Ser455Asn), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the CACNA1H gene (transcript NM_021098.3) at coding-DNA position 1364, where G is replaced by A; at the protein level this means replaces serine at residue 455 with asparagine — a missense variant. Submitter rationale: This sequence change replaces serine, which is neutral and polar, with asparagine, which is neutral and polar, at codon 455 of the CACNA1H protein (p.Ser455Asn). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with CACNA1H-related conditions. ClinVar contains an entry for this variant (Variation ID: 1004315). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on CACNA1H protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
