Uncertain significance for Spastic paraplegia — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001478.5(B4GALNT1):c.877C>T (p.Arg293Trp), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the B4GALNT1 gene (transcript NM_001478.5) at coding-DNA position 877, where C is replaced by T; at the protein level this means replaces arginine at residue 293 with tryptophan — a missense variant. Submitter rationale: This sequence change replaces arginine with tryptophan at codon 293 of the B4GALNT1 protein (p.Arg293Trp). The arginine residue is moderately conserved and there is a moderate physicochemical difference between arginine and tryptophan. This variant is present in population databases (rs769436121, ExAC 0.03%), including at least one homozygous and/or hemizygous individual. This variant has not been reported in the literature in individuals affected with B4GALNT1-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
