Uncertain significance for Sterile multifocal osteomyelitis with periostitis and pustulosis — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_173842.3(IL1RN):c.449T>C (p.Met150Thr), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the IL1RN gene (transcript NM_173842.3) at coding-DNA position 449, where T is replaced by C; at the protein level this means replaces methionine at residue 150 with threonine — a missense variant. Submitter rationale: This sequence change replaces methionine with threonine at codon 153 of the IL1RN protein (p.Met153Thr). The methionine residue is weakly conserved and there is a moderate physicochemical difference between methionine and threonine. This variant is present in population databases (rs781018527, ExAC 0.02%). This variant has not been reported in the literature in individuals affected with IL1RN-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
